You can manage a club 12 leagues down the English football pyramid. By the time you reach Premier League (assuming you can win consecutive promotions every year), all real players would be probably retired and you'd be in a fantastic football world filled with regens.
This England Level 22 database for FM19 includes real structure and rules, as well as all the cups.
It's currently complete down to level 12. And of course, it's still a work in progress. A Gargantuan task no doubt.
